IRELAND ARMY COMMUNITY HOSPITAL

Ireland Army Community Hospital, located at Building 851, 289 Ireland Ave., is an acute and general care hospital, providing managed health care for all eligible beneficiaries living on Fort Knox and in the surrounding communities. Accredited by The Joint Commission (formerly known as the Joint Commission on Accreditation of Healthcare Organizations), the hospital operates five primary care clinics, a consolidated troop medical clinic, an aviation medicine clinic and a Level II emergency room, all of which are staffed by board-certified family practice physicians, flight surgeons, and physician assistants.
Additional clinical services include but are not limited to pediatrics, ophthalmology, optometry, dental, radiology, physical therapy, occupational therapy, chiropractic clinic and pharmacy. The post’s Health and Wellness Center provides preventive health education and testing.
The mission of Ireland Army Community Hospital is to optimize wellness and military readiness by providing high quality, multi-disciplinary, evidenced based health care services in support of our Warriors and their families; past, present and future.
The dedicated health care professionals of the United States Army Medical Activity (MEDDAC) at Fort Knox, Ky., are working together to provide our patients quality health care and preventive services throughout six states. We are proud of our health care system and proud to serve our nation’s Soldiers and families.
The hub of our activity is the Ireland Army Community Hospital (IACH) located at Fort Knox which serves the Fort Knox community with primary and specialty care providers for a population of well over 30,000 Soldiers, family members, civilians and retirees.
Members of the MEDDAC team also staff a troop medical clinic (Nelson TMC). In addition, we have an Optical Fabrication Laboratory that makes glasses and mask inserts for most of the Soldiers in the eastern United States. Our Optical Fabrication Laboratory supports Force Health Protection and combat readiness of the Armed Forces by providing timely, economical, worldwide fabrication and delivery of high quality eyewear.
In addition to Kentucky, Ireland Army Community Hospital’s area of responsibility also includes Ohio, Indiana, Michigan, Illinois, and Wisconsin. Within this six state area, the MEDDAC serves the active duty population, many of whom are in isolated areas where they serve as recruiters and trainers. To serve our troops outside the Fort Knox area, we have clinics in Kentucky, Wisconsin, Michigan, Indiana and Illinois.
Much of our beneficiary health care is provided through the Department of Defense’s health care program called TRICARE. TRICARE is the military health plan that allows active duty Soldiers and their family members access to health services even though they are not near a military treatment facility or from a civilian network provider.

EMERGENCY CARE

Emergency medical services are available 24 hours a day at Ireland Army Community Hospital. In case of an emergency, dial 911 from an on-post phone. Members with non-emergency conditions should call the central appointment number 1-800-493-9602 Monday - Friday, 7 a.m. to 4 p.m.

SCHEDULING APPOINTMENTS

APPOINTMENT LINE - Ireland Army Community Hospital’s central appointment line is open from 7 a.m. to 4 p.m., Monday - Friday, except for federal holidays. To make an appointment, please call 1-800-493-9602.
In the event of severe weather conditions, you may want to contact the Appointment Center to confirm or reschedule your appointment before coming to IACH.

TRICARE ON LINE - TRICARE Prime beneficiaries can also schedule their appointments with their primary care provider using TRICARE Online’s appointment feature at www.tricareonline.com. Appointments made online can be canceled, changed, or confirmed on TOL.
TRICARE Online (TOL) is the Military Health System Patient Portal. It is designed to provide online capabilities, tools, and information for TRICARE beneficiaries.

Five good reasons why one should take advantage of using TOL:
1. You can book an appointment in the comfort of your own home. If a family member should get sick in the middle of the night or on Sunday evening you have that option to go online and book an appointment.
2. If you forget the date and time of your appointment you can log on and verify that as well. Soldiers can print off their own appointment slips to show to the commander if need be. You’ll have the option to cancel booked appointments.
3. You can also use TOL to get prescriptions refilled and have the option to check the status of your prescription. You have the option to request prescription refills from TRICARE via mail order pharmacy (TMOP).
4. It allows you to have access to your personal health records. Those who have a Common Access Card are allowed access to the TOL Personal Health Record (PHR) application during the first phase of this initiative. The TOL PHR application allows you, the MHS beneficiary, to securely access your most current Electronic Health Record (EHR) data that is stored in the Clinical Data Repository (CDR).
5. TOL allows you to transfer your account to another MTF.

DEPARTMENT OF BEHAVIORAL HEALTH

The IACH Department of Behavioral Health is located in Building 1480, off of 12th Armored Division Ave. It is located next to the Warrior Transition Battalion and across the street from U.S. Army Recruiting Command headquarters. It houses Social Work Services (SWS), Family Advocacy Program (FAP), Army Substance Abuse Program (ASAP) and the Behavioral Health Clinic.
We also provide behavioral health services to Initial Entry Training (IET) Soldiers at Nelson Clinic Annex II (Building 5980). The ASAP front desk number is (502) 626-6189. SWS/FAP front desk number is (502) 626-6170. Our Department and Behavioral Health Clinic front desk numbers are (502) 626-6188 and (502) 626-6187.
The Department of Behavioral Health provides a variety of services including, but not limited to, mental status exams, command directed evaluations, intensive outpatient groups, anger management, stress management, marital and family counseling, individual counseling as well as coordination of emergency psychiatric services when needed.
After normal clinic hours (7:30 a.m. to 4:30 p.m.) and on federal holidays and weekends, emergency psychiatric services can be coordinated through IACH’s Emergency Department. All of our services (SWS, FAP, ASAP and Behavioral Health) have daily walk-in hours. For clinic/program specific hours and procedures, please contact that clinic’s front desk.

TRICARE

The Fort Knox TRICARE Service Center (TSC) is located by the pharmacy in the basement of the Ireland Army Community Hospital at Building 851, 289 Ireland Ave. We are co-located with the Patient Service Center and offer a variety of walk-in services, including individual help with enrollments, referrals, and a self-service area that connects directly with 1-877-TRICARE or 1-877-874-2273, and various TRICARE literature. We specialize in walk-in service and are open Monday - Friday from 7:30 a.m. to 4:30 p.m. You can also find information regarding TRICARE North at www.hnfs.net/bene/home.

DENTAL CARE

The full spectrum of general dentistry care, including endodontics, oral surgery, periodontics, and prosthetics is provided for all active duty military members. The dental staff also provides educational services to post schools and assists other organizations with dental disease prevention programs.
Fort Knox has two dental clinics that provide treatment to the military community: Jordan Dental Clinic, which can be reached by calling (502) 626-8301, and the Hospital Dental Clinic, which can be reached by calling (502) 624-9670.
Routine dental services are currently not available for family members or retirees. During normal duty hours, treatment is provided at the dental clinic serving your unit.
After duty hours, Soldiers should go to the IACH emergency room.

HEALTH AND WELLNESS CENTER (HAWC)

The HAWC provides comprehensive, multi-disciplinary preventive health programs to active duty personnel, retirees and their family members, and Fort Knox civilian employees. Programs include tobacco cessation classes, weight control programs, and cholesterol and blood pressure screenings (by appointment.) Literature and video check-out services that provide customers with the latest health-related information is available.

NUTRITION

The Nutrition Care Division of Ireland Army Community Hospital strives to provide superior Soldier support and patient care through an expert group of nutrition professionals employing the full spectrum of nutrition science to optimize Soldier health, physical performance, and a nutritional education and awareness which shapes their eating habits for a lifetime.
Registered dietitians are available to provide Medical Nutrition Therapy to active-duty Soldiers, retirees and family members. Individual appointments are available by referral only. Once we receive a referral from your primary care manager, you may schedule an appointment. Referrals are not required for classes, but since seating is limited, participants must call the Nutrition Clinic for reservations.

PATIENT REPRESENTATIVE

Ireland Army Community Hospital strives to provide exceptional customer service and top-quality medical care at all times. If there is some area of concern that needs attention, there is a customer service advocate in each section, who can assist resolving those specific concerns.

PHARMACY

There are two pharmacies located on Fort Knox. The main pharmacy is located in the basement of Ireland Army Community Hospital. It is open Monday-Friday from 7:30 a.m. to 5:30 p.m., Saturday from 8 a.m. to 4:30 p.m., and training holidays from 8 a.m. to 4:30 p.m.
A satellite pharmacy is located at the Exchange. It is open Monday-Friday from 8:30 a.m. to 6 p.m., Saturday from 8 a.m. to 4:30 p.m. and training holidays from 8 a.m. to 4:30 p.m.
Both pharmacies are closed on Sundays and federal holidays.
Written prescriptions from off-post health care providers are encouraged to be taken to the Exchange pharmacy.
All prescription refills must be called in to 1-800- 440-7058 (toll-free) or (502) 624-9345 (local number), mailed in, or requested via the Internet at www.iach.knox.amedd.army.mil/pharmacy.asp ahead of time and must be claimed within seven days.
TRICARE Mail Order Pharmacy is available. The medications are directed to Express Scripts, toll-free 1-877-895-1900.

VETERINARY SERVICES

The Fort Knox Veterinary clinic is located at Building 1068 off of Spearhead Division Ave. Our hours are from 8 a.m. to 4 p.m. Monday - Friday. We are closed the last duty day of each month for inventory. You must be an ID card holder to receive services and payments are due at time of service.
